Supervision is where case decisions are made: adjusting a step, changing a strategy, or coordinating with the family. If these decisions rely only on memory, they are lost during therapist rotations, the next supervision, or meetings with the school. Supervision notes solve this.

What good notes must include

  1. Identification: child, date of supervision, type (clinic, school, home, family meeting), and attendees, with each role defined.
  2. Discussed points: goals and behaviors reviewed, with what the data showed.
  3. Decisions: what changes from now on. Only the final decision, not every idea raised during the conversation.
  4. Action items: who does what, and by when.
  5. Points for the next supervision: what needs to be observed or measured by then.

How to write in clinical style

  • Use third person and objective language: "The child responded to 7 of 10 trials independently" instead of "they did very well today."
  • Write absolute dates. "Tomorrow" or "next week" lose their meaning when the note is read a month later.
  • Assign each action to a person, by name and role.
  • Do not record spoken words that were not said or conclusions that were not reached.
  • Separate what was observed from what is a hypothesis.

Supervision notes template

Clinical supervision notes

Child: [name] · Date: [mm/dd/yyyy] · Type: [clinic / school / home / family]

Participants: [name, role]; [name, role]

Discussed points

  • [Goal or behavior]: [what the data showed during the period]

Decisions

  • [What changes, effective from when]

Action items

  • [Responsible party]: [action], by [date]

For the next supervision

  • [What to observe or measure]

Common errors

  • Notes too long: if no one re-reads them, they are useless. Prefer short bullet points.
  • Decision without a responsible party: every decision needs a name attached to it.
  • No link to data: cite the performance that motivated the decision, including the period and numbers.
  • Mixing visibility: what is internal for the team should not go to the family without review.

From recording to notes, in Proaba

In Proaba, you record supervision as a voice note, or send a video recorded on your phone. After transcription, Proaba suggests ways to turn the recording into a note, such as "Clinical Supervision Notes" or "Plan Update for the Family," and each suggestion shows the snippet that justifies it. The generated note uses absolute dates, clinical style in the third person, only the final decision, and action items grouped by the responsible party, and it knows the child, the team, and the linked family members. You review it before saving and choose who the note is visible to.
